Is 3 hours of PT every week enough after knee replacement?
What happens between your PT appointments may matter more than you think.
The 165-Hour Problem: Why We Need to Rethink Recovery After Knee Replacement
There are 168 hours in a week.
If you go to physical therapy three times a week for one hour, that's 3 hours with your physical therapist.
That leaves 165 hours on your own.
So here's the question:
Is the bulk of your knee replacement recovery determined by what happens during those 3 hours, or by what happens during the other 165?
This isn't an argument against physical therapy. Physical therapy can be an incredibly important part of your recovery.
It's simply a question of math.

Recovery Doesn’t Happen During PT Appointments
It happens in between.
You leave your PT appointment with exercises to do at home.
But recovery isn’t always straightforward.
What happens when your knee suddenly swells?
What if an exercise that felt fine yesterday suddenly feels terrible today?
What if you're not sure whether you should push through the pain, or simply rest?
Your therapist isn't there for those moments.
And that’s when the 165 hours between appointments really matter.
🎥Let's Meet Ms. Jones
Imagine this.
You just had knee replacement surgery.
You're home, taking your medication, moving around reasonably well, and doing the exercises on the handout you were given at the hospital.
Then you have your first outpatient PT appointment.
You feel pretty good going in.
And then things change.
What If You're Not the Problem?
On Friday, Ms. Jones has her range of motion measured again.
It's lower than it was on Wednesday. Her therapist asks:
"Have you been doing your exercises at home?"
And suddenly, Ms. Jones is questioning herself.
Did I not do enough?
Should I have pushed through the pain?
Did I set myself back?
But here's the bigger question:
What if she's not the problem?
Ms. Jones isn't failing.
She's trying to navigate a complicated recovery on her own.
And that's the 165-hour problem.
How Have We Improved the Rehabilitation?
Over the years, home exercise programs have moved from printed handouts to videos and apps that remind you what to do.
Those tools can be helpful.
But here's the question I've been asking:
Have we really improved the rehabilitation, or have we simply digitized the same home exercise program?
An app can show you a ‘heel slide’. But it doesn’t make the movement more comfortable when your knee is swollen, stiff, and painful.
That distinction matters.
Maybe We Need a Different Approach.
I’ve been a PT for nearly 30 years, and I’ve watched knee replacement surgery change drastically.
Surgical techniques. Implant designs. Pain management. Hospital stays.
The rehabilitation model has changed, too.
Years ago, many patients spent weeks in inpatient rehabilitation, receiving therapy 2-3 hours a day, 6 days a week.
Today, many people go home much sooner and may only see their PT 2-3 hours a week.
That made me start asking myself:
How do I need to change my plan of care to help my patients get the best possible outcome during all those hours when I'm not there?
That question is one of the reasons I created GoKnee.
